Mission:To identify and address the community's health and well-being by creating a network based on collaboration, coordination and partnering to improve the overall health of Linn County, Iowa.
Healthy Linn Care Network (HLCN) was founded in 1989 with the goal of building partnerships to improve the health and well being of citizens in Linn County. Over 100 organizations joined together to conduct the Community Health Assessment in the mid-90's. Since that time, HLCN has published two Community Health Plans – one in 1999 and, most recently, 2005. Charged with developing and maintaining collaborative networks among health care providers, businesses and community agencies, HLCN uses the Community Health Plan to identify and meet the wellness needs of all those who live, learn and work in Linn County.
Healthy Linn Care Network manages Linn County Project Access.

Hawkeye Area Community Area Action Program (HACAP) understands the importance of meeting health and nutritional needs during critical times of developmental growth. We provide a variety of services for maternal, infant and child health. The LCMS has a long history going back to 1903. It is physician driven and seeks to meet the needs of all members and the patients they serve.
We are devoted to improving the professional status of physicians, and provide a forum for positive interaction in a neutral atmosphere where members can unite as representatives of their profession. We are doctors supporting doctors. Membership is open to any physician with the M.D. or D.O. designation.
The LCMS also serves our community. We provide an active website where patients can locate physicians, and obtain health information. We offer a telephone referral service for patients.
Other activities include educational and social events for physicians, producing a community provider directory for medical offices, legislative advocacy, hosting an annual charity golf event for the free clinics, producing a domestic abuse booklet for distribution by the LC Sheriff’s Department, and we offer medical student scholarships.
We have representation on the LCMS Physician Spouse Alliance, Medical Group Managers Association, Project Access, Medical Reserve Corps, Older Adults Disaster Preparedness Planning Committee and the American Cancer Society.

Iowa Collaborative Safety Net Provider Network

Recognizing the significant role of safety net providers in providing health care services to the uninsured and underserved, the Iowa General Assembly created the Iowa Collaborative Safety Net Provider Network in 2005. Through the creation of the Network the state's community health centers, free clinics and rural health clinics work together to gather information and identify and address common challenges in providing care to the uninsured and underserved.
In 2008, a $74,970 grant was awarded to St. Luke's Health Care Foundation to support efforts to initiate Linn County Project Access.

St. Luke’s is not a building.  It is 3000 people all trying to live their mission of "providing the healthcare we’d want our loved ones to receive.”  As a four-time winner of Top 100 hospital national awards St. Luke’s consistently demonstrates itself as one of the best hospitals in the country and we strive each day to get even better.  We’ve done that for 125 years and we will do it for the next 125.  We are known for our heart and emergency services, a commitment to education, and a growing number or nationally recognized programs in women and children’s care, rehabilitation, and behavioral health.  Where we excel is not just providing better outcomes for patients in typical hospital services, but in caring for large numbers of patients in “orphan” services many hospitals do not wish to offer.  At the same time St. Luke’s consistently serves as a quality and technology innovator introducing programs like robotic surgery and physician hospitalists to our region, working each day to make St. Luke’s that “better place to be.”
